Over the past decade, HL7® FHIR® has evolved from an ambitious specification into the de facto global standard for modern health data exchange — technically mature, embedded in regulation, and supported by a growing worldwide community. Yet the hurdles that remain are structural rather than technical: fragmentation, misaligned incentives, and the long-term sustainability of the community now matter as much as the specification itself.
